We enclose here a membership application/renewal form:
  • Form in Word format (PREFERRED - Type on the form, save and e-mail as an attachment. Then we can copy and paste it, minimising errors).
  • Form in PDF format (First print, write on the form, scan, then fax or e-mail. Then we have to decipher the writing, making errors).
